Postdoctoral Scholar Appointment Forms

See the UCLA Graduate Divisions Appointment Guidelines and Forms for Postdoctoral Scholars page for the most up-to-date forms and procedures for postdoc appointments.

Research/Career Development Plan and Periodic Evaluation Template

This template is provided by UCLA Graduate Division and recommended to encourage clear understanding of the expectations of postdocs and mentors for postdoc performance.  The file contains a one-page template for the creation of a Career Development Plan that should be revisited and updated as Periodic Evaluation. This form is useful in establishing common goals and understanding, facilitating ongoing performance, and allowing both the postdoctoral scholar and mentor to evaluate performance.

Postdoctoral Scholar Annual Evaluation

This form is to be completed by the mentor prior to reappointment of a postdoctoral scholar. The mentor and postdoctoral scholar should go over the form together during an annual review meeting that includes a reevaluation of the Career Development Plan with updates for the upcoming appointment period.

Recommendation for Issuance of a Certificate of Postdoctoral Study

At the end of a postdoc's UCLA appointment, the faculty member can recommend the postdoc for a Certificate of Postdoctoral Study.  Postdocs who are approved will receive a certificate similar to a diploma to commemurate their achievement of a successful period of postdoctoral study.  This form must be completed and signed by the faculty mentor and Department Chair/Unit Head and submitted to Graduate Division for approval.
